The map seems to send the players to a random end stage each time they reach the end of the map. Experienced it once however where players reached the end but weren't being teleported anywhere, so it's possible one of the stage teleports is faulty or so. Can the end teleports be double checked, as the players reached the end and weren't teleported to go kill Death in short.
Okay, managed to replicate this one. There is a teleport at the end that just keeps bouncing you back and forth doing nothing. The logic that controls the end teleports decision is needlessly complicated so I need to spend some time untangling it; this will get looked at during the weekend - for now, I've removed the map from rotation.
