    If you're American you won't have Metal gear solid's European Extreme difficulty cause it was too difficult for USA or something. xD

    I go to the hardest available difficulty... like Gears of War 1 and 2, you have to unlock Insane so I just do Hardcore to unlock it then play through then. When I get fallout I'll be on the hardest difficulty probably. :D
